More about the survivorship plans

Last week we shared a new tool from OncoLife which allows patients to created their own survivorship plans. One of our members wrote about his experience......And we had a doctor also contact us for more information. This docter had a patient who read about it on our site talked to him about it. He was curious and wanted to know more.

It is great to have clinicians and patients working together to improve survivorship information and communication.
Who new that technology was going to be so useful for those affected by HNC!

If you have not yet created your own survivorship plan you can do so by accessing the link.

  • It has taken from 2013 till now to find out that my partner is entitled to be “enrolled” in a Ministry of Health plan called “Care Plus”.  Find out if you are entitled to “Care Plus” ask your GP, specialist or hospital, here is the link for more information

On this subject.  Unless you have ALL the technical information it is almost impossible to fill in this online form.  It seems to me that it needs to be filled in with your doctor.  Also (as a carer) the information required is "fuzzy" about sex, age, etc as it does not seperate carer information from patient information - too vague.
